Our patient list is currently closed (approval received from Swindon CCG), therefore we are not registering new patients at this time. 

This position will be reviewed in 6 months (January 2020)

If you want to request access to your electronic medical records and you have not yet registered for online access you will need to call into the Practice with two forms of identity such as passport and utility bill.

Once you have done this and have your username and password please click on the document links below to print off a Patient Information Leaflet, and Access OR Proxy Access form which must be completed and submitted to the Practice before access can be considered.  

Please note that due to the requirement to review all medical records before access is granted there will be a delay of approximately one month

The doctors and staff at North Swindon Practice in Haydon Wick are proud to offer the highest standard of patient-centred healthcare. 

Disabled Access


All consulting and waiting areas are on the ground floor and have wheelchair access. Any member of staff will be happy to provide assistance if required.


There are lots of free parking spaces available, with 2 disabled parking bays reserved for blue badge holders. Please leave these free for our disabled patients.

GP Training

North Swindon Practice is a GP Training Practice.  Trainees may include GP Registrars (doctors completing their final training to become a GP) and Foundation Year Two doctors (qualified doctors who, as part of their training, will work under the supervision of one of the partners).

Patients may book appointments with our junior doctors.  You may be asked if you mind having your consultation observed or videoed for training purposes, but you can refuse or withdraw your consent before, during or (in the case of the video) after the consultation.
